Booking your new-patient consultation
New patients begin with a one-hour consultation with a medical provider. You can call or text us at (817) 533-0988, or book online.
Your consultation is time to talk through your symptoms, history, and goals. If testing is right for you, we'll explain what's involved. Some treatments are not done on the same day as your first visit, especially when lab work is needed first.
Please complete your new-patient paperwork at least 48 hours before your appointment, and bring a valid government-issued photo ID to every visit.
Consultation fee
All new patients pay a non-refundable consultation fee, which can be credited toward treatment. Your appointment is confirmed once payment and the required paperwork are received. Fee amount and terms to confirm
Cancellations, rescheduling, and missed appointments
- Please cancel or reschedule at least 24 business hours in advance. Late cancellations and no-shows carry a $100 fee.
- Arriving more than 10 minutes late may shorten your appointment or require rescheduling.
- Two or more no-shows in 12 months may require a larger deposit for future visits.
- Unused sessions in packages or prepaid treatments expire after 12 months unless otherwise specified, and can't be transferred to another patient.
Payment and financing
Revique is a cash-pay practice and does not bill insurance. Payment is due at the time of service. Itemized receipts are available on request if you'd like to submit them to your insurer, HSA or FSA, or tax professional; reimbursement isn't guaranteed.
We accept cash, certified checks, Visa, Mastercard, American Express, Discover, and HSA/FSA cards. A surcharge applies to credit and debit card transactions.
For long-term hormone, weight, and intimate health care, ask about the Revique Care Management Program: ongoing provider oversight between visits for a monthly fee, with savings on eligible services and access to in-house financing.
Refunds
Payments, deposits, consultation fees, and program fees are non-refundable and non-transferable. Refunds are considered only in rare cases, such as a verified medical contraindication or an error by our staff.
Prescriptions and treatment clearance
Every patient is evaluated and medically cleared by a Revique provider before any procedure. As required by law, prescriptions and treatments need an up-to-date annual Good Faith Exam. Prescriptions are issued at the provider's discretion, and controlled substances are not routinely prescribed.
Your privacy
Your health information is protected. Revique follows HIPAA privacy requirements, and you may request our Notice of Privacy Practices during your visit. See our full Privacy Policy for details.
